If your organization is among the 25% of U.S. hospitals participating in the new APM (alternative payment model), you’ve got your work cut out for you.
By January 2026, select hospitals must participate in the Transforming Episode Accountability Model (TEAM) initiated by CMS/CMMI. If your organization is selected, you’re responsible for overseeing the full spectrum of care for Medicare patients undergoing surgical procedures. Oversight of care begins upon commencement of an outpatient procedure or hospital admission and continues until 30 days post-procedure or after a patient leaves the hospital.
TEAM is a coordinated effort between CMS and providers that seeks to:
- Improve health outcomes and value-based care for Medicare patients
- Decrease hospital readmittance and adverse outcomes
- Reduce costs for patients and providers alike
But it won’t be easy. TEAM participating providers and health systems have a lot of work ahead.
