According to the National Library of Medicine, more than 75% of U.S. adults report having one or more chronic health conditions. And while programs exist to increase the use of preventive care, such as Medicare Advantage Wellness Visits, they often go unused—not due to a lack of programs, but a lack of precision engagement. People simply aren’t getting the right message at the right time in the right way.

When serious chronic or life-threatening conditions are caught and treated early, it keeps more people out of the hospital—improving lives while reducing systemic waste. Examples:

  • Patient Acquisition through Preventive Heart Care: A large U.S.-based health system wanted to engage patients to stay on top of heart disease risk factors and prevent future potentially life-threatening heart-related emergencies. Carenet worked with the provider to conduct targeted outreach via phone and digital channels, encouraging current and prospective patients to schedule free and vital preventive screenings. As a result, the client saw a 150% increase in preliminary heart scans—36% of which came from new patients, contributing directly to strategic growth and stronger outcomes.
  • Pre-Diabetes Care Changes Lives: A managed health services company that helps people access Medicare, Medicaid, and other state-driven insurance offerings crunched the numbers on prediabetes care. According to the National Library of Medicine, the total annual cost for prediabetes management can range from $1,000 – $2,500, but patients who delay early intervention and progress to a Type 2 diabetes diagnosis can see costs up to 50x higher with worse health outcomes. Through working with Carenet, the company conducted nearly 350,000 health risk assessments in just one year. These assessments helped at-risk patients identify prediabetes symptoms early, take control of their health trajectories, and prevent a future diabetes diagnosis.
